Accident and Incident Investigation

Overview
The ACI Accident and Incident Investigation course will explore the fundamentals of accident and incident prevention and investigation. It guides learners on how to employ defenses to prevent accidents and incidents from occurring and how to approach and conduct the accident and investigation process at their airport.

Objectives
On completion of this course participants will be able to:

  • Recognize the major causes and serious consequences of accidents and incidents
  • Understand defenses that can be employed at the airport to prevent accidents and incidents from occurring
  • Appreciate how effective accident and incident investigation contributes to safety
  • Understand how to undertake a professional investigation at the airport, including how to:
    • Gather and retain evidence
    • Apply effective interview techniques
    • Interpret the facts
    • Determine underlying causes and contributing factors
  • Describe the role and responsibilities of an investigator
  • Recognize the personal qualities and knowledge, skills and abilities (KSAs) required by investigators and the importance of training to ensure competence
  • Understand the importance of providing a detailed, fact-based report and the required contents
  • Understand how to monitor investigation outcomes

Accident and Incident Investigation

  • The purpose and benefits of investigations.
  • Steps in the investigation process:
    • Commencing the investigation.
    • Gathering and retaining evidence, including applying effective interview techniques.
    • Analysing information and determining underlying causes and contributing factors.
    • Making safety recommendations.

The Accident Investigator

  • The role and responsibilities of an investigator.
  • The personal qualities and knowledge, skills and abilities (KSAs) of effective investigators.
  • Training requirements for investigators.

Post Investigation Reporting and Action

  • Reporting requirements.
  • Monitoring investigation outcomes.
  • Following up to ensure corrective actions are implemented.

Assessment

This course includes an integrated online assessment.

Candidates must successfully complete the assessment to receive a Certificate of Completion.
